Service Procedure

1.Fill out the RMA Request Form and submit it. Download the RMA Form
You may attempt to request RMA by downloading the form itself and fax it to 908-548-6106 or email service@Fosa.com.
2. An RMA number will be sent to you (via email on most circumstances) upon receiving your completed RMA Request form.
3. NO RMA number will be issued without a valid Serial Number and Invoice Number.
4. Backup all your valuable data. FOSA will not be responsible for the contents of your storage devices.
5. Carefully pack your computer and write the RMA number on the outside of the box (best if use original box)
6. Make sure you send items listed in RMA Request form.
7. Mail the machine to the following address:

Service & Repair Center, Unit C

South Plainfield, NJ 07080

Note: FOSA will not be responsible for RMA items damaged by carriers

Packaging Advice
·UPS, Federial Express, or other carriers provide varity shipping methods. No matter which service you decide to use, we strongly suggest that you purchase insurance for the package to reduce the risk if damage happened during the shipping. Never use the "just-fit" size of box to ship your machine. Use the box that double the dimesion of the machine at least. Fill the package with Industrial Peanuts, Air Bubble, or Soft Foam (at least 1" thick) to protect the machine. NEVER use newspaper to wrap the machine. They will not provide enough protection for your machine.
